The best way to begin your path to optimal health is to call us. Our staff will be happy to answer any questions that are not answered on the web site. We are available to answer your calls Monday through Friday at our office.

We are located in Independence Ohio, the crossroads of greater Cleveland, and are easily accessible by I-77 and I-480. Call to schedule your consultation.

HOURS:

• Monday- 9am to 6:30pm
• Tuesday- 9am to 5pm
• Wednesday- 9am to 5pm
• Thursday- 11am to 6:30pm
• Friday- 9am to 1:30pm

Our patients are screened with a detailed personal, medical, family, medication & allergy history in addition to comprehensive laboratory testing and a full physical. Dr. Kocka will structure an individualized treatment plan that will fit your unique profile. These include Bio-Identical Hormones (FDA-approved hormones identical to your body's hormones), nutritional supplements, herbal & vitamin prescriptions, guidance with diet plans and exercise programs. If you are not a candidate for certain hormone replacement, we will inform you based on your laboratory results and symptoms. This program is not for everyone, it is for those patients who are concerned with optimal function and health.

As an additional benefit, Dr. Kocka can be your primary care doctor as well as your Age Management physician. Dr. Kocka is happy to provide primary care and acute medical care outside of this program billed to the patient’s insurance. We accept all major insurance, including medicare for general care outside the program. (www.premierhealthclinic.org)

Insurance:

Unlike many other programs, the first two visits are typically covered under your insurance. We accept all major insurance, Medicare and Medicaid excluded. Your program will begin only after your screening visits with the physician to determine if you are a candidate that would be benefited by our Age Management program. The only obligation is your co-pay, so there’s no reason not to get tested. If you are not a candidate for hormone replacement therapy, you will not receive any additional bills for the consult and we would be pleased to see you as a general patient outside the Age Management program.

Outside Cleveland/Akron:

If you are a patient outside the Cleveland/Akron area, call us to arrange an altered visit schedule. We will work with you to coordinate in-office and phone visits.

Fees:

We charge an annual fee for the Age Management Program, with discounts for renewals. Your first two screening visits are covered by your insurance. Call for program annual fee.

Prescriptions:

You can take our prescriptions for hormones and fill at any licensed pharmacy, though we obtain a group rate from a nearby pharmacy. Hormones (other than HGH) are bio-identical, generic, and relatively inexpensive. Vitamins & Herbals may be purchased through Premier Age Management Group Pricing for pharmaceutical grade products or purchased over the counter.

Laboratory Testing:

Laboratory studies are typically covered by insurance with appropriate symptoms and diagnoses. Any patient without insurance will be granted our clinic discount at Labcorp Laboratories. We can draw your blood on sight. We submit a long list of tests to provide us detailed information about your hormones and general health.
